Is the result truly anonymous?

No. Consistent replacement is pseudonymisation, which GDPR still treats as personal data, because anyone holding the mapping or a matching external dataset can reverse it. Genuine anonymisation requires removing enough information that re-identification is impossible — usually including aggregation or generalisation, not just replacement.
